Brooklyn Game Lab Parent/Guardian Agreement & Notice of Policies
I represent that I am the parent or legal guardian of the child who will be participating in a semester, summer lab, program or other event (hereinafter the “Program”) by or with Brooklyn Game Lab LLC (hereinafter “Game Lab”).
I hereby certify that my child is in good health and that my child has no limitations that with or without a reasonable accommodation would preclude his/her safe participation in the Program. I acknowledge and agree that my child should not enter and participate in the Program unless medically and physically able.
I agree not to bring my child to Game Lab programming if my child exhibits symptoms of illness; if my child is subject to quarantine; or if other current guidelines preclude my child’s participation due to an infectious disease risk.
I hereby give my authorization and approval for my child’s participation in any and all of the activities of the Program. I further acknowledge and agree that participation in the Program involves outdoor activities (known as “Battle Lab”) in which children participate in LARP (live action role play) activities, and, further, that risks include physical injury.
While Game Lab abides by recommended guidelines regarding health and safety, I recognize that there are certain inherent risks associated with participating in the Program, and I accept such risks.
I further acknowledge that the above is not inclusive of all possible risks associated with the Program and that the above in no way limits the extent or reach of this release.
Health & Safety Policy
Brooklyn Game Lab has, and will continue to, adhere to recommended guidelines regarding health and safety, which may include conducting health checks, use of masks, and social distancing. Staff and program participants are encouraged to adhere to current vaccination recommendations.
Brooklyn Game Lab adheres to robust cleaning procedures which include regular disinfecting of surfaces and professional cleaning of the Lab spaces. Regular hand washing or use of hand sanitizer is encouraged for all program participants and disinfectant wipes are available for use as needed.
Allergies & Medications
I hereby acknowledge that I’ve alerted Game Lab (via the Program registration form and at frontdesk@brooklyngamelab.com) of any medical issue, dietary issue, allergy, or illness that could impact the safety and well being of my child and their peers in the Program.
If my child might require an EpiPen, other medical device or medication, I have provided that (labeled with my child’s name) in my child’s bag with explicit, written instructions and will notify the Game Lab staff at drop-off.
While Brooklyn Game Lab strives to accommodate children with medical application needs, staff do not provide or administer medications and are not responsible for reminding children to administer their medications.
Managers are trained in First Aid and EpiPen administration and, in the event of a life threatening emergency, EpiPens may be administered to children who have them, if indicated on their registration form.
Emergency Medical Treatment
I do hereby give permission to Game Lab to obtain the necessary emergency medical treatment for my child, with the understanding that the family is notified as soon as possible. If my child receives an injury during the session, I am responsible for any medical expenses incurred.
School Day and After School Registration/Early Termination
By enrolling in Game Lab’s School Day or After School Programs I am committing to my child’s participation for the full semester, and agree that my credit card will be charged monthly over the course of the semester. If I must terminate my child’s participation at any time during the semester, my child’s Game Lab participation will end and I agree to pay the early termination fee. The early termination fee is $20 for every month enrolled in a semester, after the first month of enrollment.
School-Year Semester Programs Make-Up Policy
School-Year Semester programming (ie. After School, Social Skills, Executive Functioning) is not held on public school holidays, certain holiday weekends, and other school closure days (including “snow days.) These closures are factored into overall semester fees and there are no make-ups for days when programming is not scheduled. Make-ups can be requested for sessions missed due to illness or family scheduling issues. I understand Game Lab may not accommodate a make-up beyond the week the absence occurs and that credits are not issued for missed sessions.
Weekend Drop-In Programs Make-Up Policy
Weekend Drop-In programming is purchased by the day and participants select and schedule their attendance dates in Sawyer, from the calendar of available dates. Weekend Drop-In programming is generally offered every weekend, except on certain holiday weekends. If a date is selected and scheduled by a participant who subsequently is unable to attend at that time, Game Lab may accommodate a make-up, depending on availability. To make arrangements please email frontdesk@brooklyngamelab.com. Credits are not issued for missed sessions.
Summer Lab and School Holiday Registration Policy
To secure a seat, payment must be received in full at time of booking. I understand there is a 48-hour grace period in which to cancel a reservation and receive a refund, except when the seat is booked for an activity that occurs within those 48 hours, in which case, there is no refund. There is a 5% processing fee for all refunds.
If canceling a reservation beyond the 48-hour grace period, Game Lab may transfer your reservation to a future date, pending availability. Alternatively, credit may be issued. If a reservation is canceled within seven days of the activity date, credit will be issued less $20 for each date canceled. Credits are available for use for 2 years following issuance and may be applied towards any Game Lab program. I understand there are no refunds for sales after the 48 hour grace period.
Lunch/Snack Policy
I agree to provide my child with lunch (for full day programs) and snacks. All food must be tree-nut free and not require refrigeration. If my child forgets their lunch and/or snacks or their lunch and/or snacks do not meet the requirements set above and Game Lab has to provide my child with lunch/snacks, I agree to fully reimburse Game Lab for the cost of the food.
My child will bring a water bottle when attending Game Lab.
Cell Phone & Electronic Devices Policy
Game Lab is all about unplugged gaming! In-person Game Lab programming does not require children to use electronic devices at any time. I understand that children coming to Game Lab with electronic devices, including cell phones and laptops, are expected to keep these devices in their bags or jacket pockets. The safety and security of these devices are the responsibility of the child. These devices should not be visible or be heard in the Lab during programming, and facilities are not provided to charge them. Upon conclusion of programming, children may use their devices as needed. If I need to reach my child during programming, I may do so most effectively by calling the Lab directly at 718-788-1122. If necessary, children may use their devices to contact parents or guardians during programming, with permission from, and under supervision of, staff.
Outdoor Gear
I understand Game Lab may conduct programming outdoors and I agree to equip my child with appropriate gear for outdoor conditions. I understand that if I would like my child to wear sunscreen while outside, I will provide that in their bag each day. Game Lab staff are not responsible for children’s use of sunscreen and cannot apply sunscreen to a child. For days where it may rain, I agree to provide appropriate rain gear for my child.
Design Release
I understand that my child will be participating in large, collaborative game design efforts and acknowledge that any contribution, play-testing feedback or design suggestions offered up as part of the Program are property of Game Lab and eligible for use in Game Lab programming, marketing materials and product line.
Photo Release
I permit Game Lab to use photographs of my child and/or my child’s projects and work in Program materials. I understand that personal information about my child will never be shared in conjunction with these photographs without my permission.
Communication Policies
I understand that in the event Game Lab staff must contact a child’s guardian, the contact details provided in Sawyer upon registration, will be used. This may include phone calls, texts or emails, depending on the nature of the notification. I permit contact information provided in Sawyer to be accessed and used for this purpose. Program promotional information may also be shared via email addresses on file, unless instruction is provided in writing to frontdesk@brooklyngamelab.com, to be removed from such distributions.
Other Policies
I understand that Game Lab reserves the right to ask my child to leave the session if it is determined that he/she is in any way disruptive to the Program. Any decision made by Game Lab in this regard will be at Game Lab’s sole discretion. Children exhibiting behavior that may be harmful to themselves or others may be gently held by supervising staff to prevent them from causing harm. Children who do not meet the age requirements for a session may be asked to leave. I understand that if my child is asked to leave a Game Lab session for any reason, no refund will be issued. I agree to pay for all damages to the property and facilities of Game Lab caused by my, or my child’s negligent, reckless, or willful actions.
I understand that if Game Lab is forced to cease or adjust activities due to changes in regulations, operating guidelines, weather conditions or for any unforeseen circumstance, Game Lab will strive to provide alternate programming which may include providing on-line programming in lieu of in-person activities. When substituted programming options are available, Game Lab’s cancellation policy remains in effect.
By permitting my child to participate in the Program, I agree to release, indemnify and hold harmless on behalf of myself, my heirs, representatives, executors, administrators and assignees Game Lab, its officers, agents and employees from any cause of action, claims, or demands of any nature whatsoever, including but not limited to a claim of negligence, which I, my heirs, representatives, executors, administrators, and assignees may now have or have in the future against Game Lab, on account of any personal injury, property damage, death or accident of any kind arising out of or in any way related to my child’s enrollment in the Program.
I have read and understand the above information and agree to its terms.
Through my signature below, or checking the appropriate box during online registration, I acknowledge that I have read this Agreement and understand that it includes an assumption of risk and a release of liability. I understand that Game Lab is relying on this waiver to allow my child to participate in the Program.
