Brooklyn Game Lab Parent/Guardian Agreement
I represent that I am the parent or legal guardian of the child who will be participating in an After school, weekend, holiday, summer or other, program or event (hereinafter the “Program”) by or with Brooklyn Game Lab LLC (hereinafter “Game Lab”).
I hereby certify that my child is in good health and that my child has no limitations that with or without a reasonable accommodation would preclude his/her safe participation in the Program. I acknowledge and agree that my child should not enter and participate in the Program unless medically and physically able.
I agree not to bring my child to Game Lab programming if my child exhibits symptoms of illness; if my child is subject to quarantine; or if other current guidelines preclude my child’s participation due to an infectious disease risk.
I confirm my child is enrolled in age-appropriate programming, as defined by Game Lab during the Program enrollment process. I accept that children who do not meet the age requirements for a session may be asked to leave.
I confirm that I have disclosed all pertinent information regarding my child’s past school experience and/or behavior, and have brought to Game Lab’s attention special needs, behavioral, physical or other concerns that should be considered to determine if the Program is appropriate for my child. This information was communicated by email to frontdesk@brooklyngamelab.com.
I understand that Game Lab reserves the right to ask my child to leave the session if it is determined they are disruptive to the Program. Any decision made by Game Lab in this regard will be at Game Lab’s sole discretion. Game Lab takes Physical and Emotional safety seriously, and staff may intervene to stop children from getting physical if they become a danger to themselves or others. I accept that if my child is asked to leave a Game Lab session for any reason, no refund will be issued. I agree to pay for all damages to the property and facilities of Game Lab caused by my child, or my child’s negligent, reckless, or willful actions.
I hereby give my authorization and approval for my child’s participation in any and all of the activities of the Program. I acknowledge and agree that participation in the Program may involve outdoor activities, including “Battle Lab”, in which children participate in LARP (live action role play) activities.
While Game Lab abides by recommended guidelines regarding health and safety, I recognize that there are certain inherent risks associated with participating in the Program, including physical injury, and I accept such risks.
I further acknowledge that the above is not inclusive of all possible risks associated with the Program and that the above in no way limits the extent or reach of this release.
Allergies & Medications
I hereby acknowledge that I’ve alerted Game Lab (via the Program registration form and at frontdesk@brooklyngamelab.com) of any medical issue, dietary issue, allergy, or illness that could impact the safety and well being of my child and their peers in the Program.
If my child might require an EpiPen, other medical device or medication, I have provided that (labeled with my child’s name) in my child’s bag with explicit, written instructions and will notify the Game Lab staff at drop-off.
I accept that while Game Lab strives to accommodate children with medical application needs, staff do not provide or administer medications and are not responsible for reminding children to administer their medications.
I understand that Game Lab Managers are trained in First Aid and EpiPen administration and, in the event of a life threatening emergency, EpiPens may be administered to children who have them, if indicated on their registration form.
Emergency Medical Treatment
If my child receives an injury during the Program, I hereby give permission to Game Lab to obtain necessary emergency medical treatment for my child, with the understanding that I, or emergency contacts provided upon registration, are to be notified as soon as possible., I accept that I am responsible for any medical expenses incurred.
Lunch/Snack Policy
I agree to provide my child with lunch (for full day programs) and snacks. If my child forgets their lunch and/or snacks or their lunch and/or snacks do not meet the requirements set above and Game Lab has to provide my child with lunch/snacks, I agree to fully reimburse Game Lab for the cost of the food.
All food must be nut free and not require refrigeration.
On hot days Game Lab may offer program participants a frozen treat (i.e ice pop) during outdoor activities. I have informed Game Lab during the program registration process, if my child may not receive a frozen treat.
My child will bring a water bottle when attending Game Lab.
Outdoor Gear
I understand Game Lab may conduct programming outdoors and I agree to equip my child with appropriate gear for outdoor conditions. Game Lab recommends close-toed shoes. I understand that if I would like my child to wear sunscreen while outside, I will provide that in their bag each day. Game Lab staff are not responsible for children’s use of sunscreen and cannot apply sunscreen to a child. For days where it may rain, I agree to provide appropriate rain gear for my child.
If my child should not participate in outdoor programming I agree to communicate this in writing to frontdesk@brooklyngamelab.com prior to my child attending programming.
Cell Phone & Electronic Devices Policy
Game Lab is all about unplugged gaming! Game Lab programming does not require children to use electronic devices at any time. I understand that children coming to Game Lab with electronic devices, including cell phones and laptops, are expected to keep these devices in their bags or jacket pockets. The safety and security of these devices are the responsibility of the child. These devices should not be visible or be heard in the Lab during programming, and facilities are not provided to charge them. Upon conclusion of programming, children may use their devices as needed.
If I need to reach my child during programming, I may do so most effectively by calling the Lab directly at 718-788-1122.
Personal Property
Program participants are solely responsible for any personal items they choose to bring to Lab, including but not limited to clothing, toys, trading cards, sports equipment, cell phones and other valuables. Game Lab facilities are not equipped to securely store personal items. Items may be stored in shared spaces and are not individually locked or monitored. We strongly recommend that valuable or sentimental items be left at home and all personal items sent with your child be clearly labeled with your child’s name.
I understand that if my child brings Trading Card Game (TCG) cards (e.g., Pokémon, Magic: The Gathering, etc.) to the Lab, these are their personal property and they are responsible for their safekeeping. Cards should be in a container (deck box, ziplock, binder, etc.) and labeled with the child’s first and last name. Game Lab is not liable for any lost, stolen, or damaged cards and is not responsible for monitoring card trades, usage, or handling. Children bring TCG cards at their own risk. If children choose to trade cards or play games with others, they do so voluntarily and with parental permission. All Program participants are expected to be respectful, honest, and fair when interacting with others; poor behavior during TCG activity may result in a request to put cards away or be excluded from card-related activities. If you do not wish for your child to participate in TCG-related activities, please keep the cards at home.
I accept that Game Lab is not responsible for any lost, stolen, or damaged personal property.
Late Pickup
I understand that I am responsible for pickup up my child promptly at the end of the Program. If I, or my child’s caregiver, will be late, I will call and notify Game Lab of the estimated time of arrival. I acknowledge that if my child is not picked up within ten minutes after the conclusion of the Program, Game Lab may call all numbers listed in my registration profile to discuss pick up arrangements. I accept that after a grace period of ten minutes, a fee of $1 per minute may be charged to my card on file.
Design Release
I understand that my child will be participating in large, collaborative game design efforts and acknowledge that any contribution, play-testing feedback or design suggestions offered up as part of the Program are property of Game Lab and eligible for use in Game Lab programming, marketing materials and product line.
Photo & Video Release
I permit Game Lab and its representatives to take photographs, video recordings, or other media of my child while participating in Game Lab programs and related activities. I authorize Game Lab to use, reproduce, and publish these images and recordings for any lawful purpose related to promoting Game Lab. This may include (but is not limited to) use in: Social media posts (e.g., Facebook, Instagram, YouTube); Printed materials such as brochures or flyers; Game Lab’s website and newsletters; Public presentations and reports. I understand that these materials may be used indefinitely and without compensation. I also release Game Lab and its representatives from any liability connected with the use of my child’s image or voice as described above.
Communication Policies
I understand that in the event Game Lab staff must contact a child’s guardian, the contact details provided in Sawyer upon registration, will be used. This may include phone calls, texts or emails, depending on the nature of the notification. I permit contact information provided in Sawyer to be accessed and used for this purpose. Program promotional information may also be shared via email addresses on file, unless instruction is provided in writing to frontdesk@brooklyngamelab.com, to be removed from such distributions.
Other Policies
I understand that Game Lab operates four locations along 7th Avenue in Park Slope, between 8th and 17th (Windsor Terrace) Streets and that to ensure the best gaming experience for all participants, children may be walked by Game Lab staff between locations or to Prospect Park, for outdoor programming.
I understand that if Game Lab is forced to cease or adjust activities due to changes in regulations, operating guidelines, weather conditions or for any unforeseen circumstance, Game Lab will strive to provide alternate programming which may include providing on-line programming in lieu of in-person activities. When substituted programming options are available, Game Lab’s cancellation policy remains in effect.
By permitting my child to participate in the Program, I agree to release, indemnify and hold harmless on behalf of myself, my heirs, representatives, executors, administrators and assignees Game Lab, its officers, agents and employees from any cause of action, claims, or demands of any nature whatsoever, including but not limited to a claim of negligence, which I, my heirs, representatives, executors, administrators, and assignees may now have or have in the future against Game Lab, on account of any personal injury, property damage, death or accident of any kind arising out of or in any way related to my child’s enrollment in the Program.
I have read and understand the above information and agree to its terms.
Additionally, I have read and understand relevant Program-specific policies (After School, Holiday & Summer Labs and Weekend Programs), and am aware of Game Lab’s Health & Safety and Privacy Policies, all of which are posted in the footer of the Game Lab website at www.brooklyngamelab.com
Through my signature below, or checking the corresponding box during online registration, I acknowledge that I have read this Agreement and the relevant Program Policies, and understand these include an assumption of risk and a release of liability. I understand that Game Lab is relying on this waiver to allow my child to participate in the Program.
